hiring a Home Health Occupational Therapist with at least 1 year of
OT experience and a background in home health for a day shift,
Monday–Friday contract position with rotating weekends. /n /n
Responsibilities/n - Home health setting serving a primary care
patient population - Day shift, 8:00 AM–5:00 PM, Monday through
Friday with rotating weekends - 8-hour shifts, 5 days per week -
Mileage between patient visits is reimbursed - Provide occupational
therapy services to patients in their homes as part of a home-based
primary care program - No local candidates within a 50-mile radius
of the facility will be considered /n /n Requirements/n - Current,
active OT license (license required if applicable) - Minimum 1 year
of experience as an Occupational Therapist - Home health experience
required - Current BLS certification required - Illinois state
license required - Candidates must reside more than 50 miles from
